Job summary

Advocate Health Care is seeking a Hospice Admissions Nurse (Full-Time, Evening Shift) to serve Condell Medical Center and Lutheran General Hospital. You will admit patients to hospice services, lead the interdisciplinary team on your caseload, and coordinate care and communication with families and clinicians.

The role requires an RN license in the state, BLS certification, and the ability to travel within the Advocate Aurora service area.

Qualifications

Registered Nurse license in the state where the team member practices.
Basic Life Support (BLS) for Healthcare Providers certification by the AHA.
May obtain within 6 months based on department discretion.
Valid driver’s license issued by the Division of Motor Vehicles.

Responsibilities

Admission to hospice services.
Lead the interdisciplinary care team for each patient and family.
Obtain history and perform physical assessment.
Assess appropriateness for hospice services and level of care; collaborate with attending physician and IDT.
Explain hospice services to patients/families and obtain informed consent.
Coordinate services and communicate with IDT for handoffs.
Develop and implement the multidisciplinary plan of care; reassess as needed.
