Job summary

Advocate Health is seeking a Hospice Registered Nurse to provide compassionate after-hours care to hospice patients in their homes and community settings. The role involves triage, symptom assessment, and supportive education for patients and families.

You will collaborate with physicians, APPs, and the interdisciplinary team to coordinate care, document actions in the EMR, and ensure adherence to hospice goals and safety standards.

Qualifications

• Completion of an accredited nursing program.
• Active RN license in the state of practice.
• BLS certification – American Heart Association.
• 1 year of Hospice or related clinical experience.
• Willingness to travel to patient homes.

Responsibilities

• Provide after-hours triage, assessments, and interventions for hospice patients.
• Respond to calls; educate and determine interventions aligned with care goals.
• Conduct in-person visits to manage symptoms and ensure comfort.
• Manage end-of-life symptoms including pain, dyspnea, agitation.
• Initiate physician orders and hospice protocols within scope.
• Document encounters timely in the EMR; communicate changes to team.
• Collaborate with physicians and interdisciplinary team.
• Adhere to policies, regulations, and safety initiatives.
• Provide emotional support and family education.
