Job summary

Advocate Health in Illinois seeks a part-time Hospice Registered Nurse to support the Supportive Care Team with after-hours patient care. You will perform nursing assessments, triage, and interventions for hospice patients, coordinating with a multidisciplinary team to align with goals of care.

The role requires weekend coverage (Saturday and Sunday, 8am–8pm) and travel to patient homes. Strong communication, critical thinking, and EMR documentation are essential for safe, compassionate care.

Qualifications

• Active RN license in Illinois
• BLS certification - American Heart Association
• One year of nursing experience in any clinical setting

Responsibilities

• Provide comprehensive nursing assessment, triage, and interventions for hospice patients during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms, provide education, and determine appropriate interventions.
• Conduct in-person visits to manage acute symptom changes and ensure patient comfort and safety.
• Document all patient encounters, assessments, interventions, and communications accurately in the EMR.
