Job summary

UnitedHealth Group is seeking an experienced RN Admissions Nurse Hospice to guide patients and families through the admission process, assessing physical, psychosocial, and spiritual needs to establish an individualized plan of care. The role requires collaboration with physicians, facilities, and the interdisciplinary team to coordinate services and educate clients and families.

As part of the Optum family, you will travel between facilities, perform skilled interventions, and ensure comfort

Qualifications

• Current RN license for applicable state(s).
• Current CPR certification.
• Minimum 2+ years of clinical RN experience in acute care.
• Valid driver's license and reliable transportation.

Responsibilities

• Admit hospice patients by assessing initial physical, psychosocial, and spiritual needs.
• Coordinate initial plan of care with physician and interdisciplinary team.
• Provide skilled interventions and caregiver instructions for symptom control.
• Serve as liaison between disciplines and facilities during admissions.
• Document care progress accurately and timely per policy.
• Communicate patient changes to the clinical team and obtain physician orders as needed.
• Fulfill on-call responsibilities as assigned.
