Job summary

AccentCare is seeking a dedicated Admissions Coordinator in Rosemont, IL, for a full-time role. You will handle referrals, gather essential patient data, and coordinate with the care team to initiate hospice services.

The position follows a Monday–Friday 8am–5pm schedule with some weekends and holidays required, and offers comprehensive benefits. The ideal candidate has an associate degree (preferred), 1+ year hospice experience, and strong communication and organizational skills to support

Qualifications

• Associate degree preferred.
• 1+ year hospice experience.
• Excellent written and verbal communication skills.
• Strong organizational skills.
• Excellent customer service background.
• Familiarity with hospice policies and regulatory requirements.

Responsibilities

• Receive professional referrals and collect referral info for EMR entry.
• Input referrals and basic patient data into the EMR system.
• Coordinate and schedule visits with staff and physicians.
• Communicate with referral sources and Seasons staff to initiate hospice services.
• Maintain ongoing communication during sign-on, consent and admission phases.
• Keep EMR documentation and referral statistics up to date.
