Qualifications

• Current MD nursing license required.
• Three years of nursing experience preferred.
• Home Health or Hospice experience beneficial.

Responsibilities

• Completes initial assessment of patient and family/caregivers.
• Implements the initial care plan with Medical Director approval.
• Provides direct nursing care to maintain the highest level of care.

Skills

Knowledge of universal precautions for infection control

Ability to provide patient/caregiver education

Education

Diploma, ADN and/or BSN from an accredited institution
