VIA Health Partners in Charlotte, NC is seeking an RN to present hospice services at admission, coordinating with patients, families, physicians and the interdisciplinary team to develop specific care goals. This full-time day-shift position requires an active NC/SC RN license, at least 2 years of related experience, and a 2-year degree.

Qualifications

• A Diploma or Associate’s Degree in nursing required; Bachelor’s Degree preferred.
• Current, active, and unrestricted NC/SC license as Registered Nurse (RN).
• Minimum 2 years related experience. Experience in direct care of patients with life limiting illnesses required.
• Computer proficiency is required.

Responsibilities

• Coordinate patient/caregiver care and develop plan of care.
• Conduct admission nursing assessment and identify patient/caregiver needs.
• Communicate with the interdisciplinary team to ensure care delivery.
• Maintain accurate clinical documentation and medication profiles.
