Job summary

Elite Hospice, part of LHC Group within the Optum family, is hiring a Hospice Admissions Nurse. You will manage the admission process and ensure regulatory compliance while advocating for patients and families.

Responsibilities include assessing eligibility, coordinating with physicians and referral sources, and documenting admissions. A compassionate approach and strong teamwork are essential to support patients in home settings.

Qualifications

• Current unrestricted RN license in the state of operation.
• Valid driver's license and access to a dependable vehicle.

Responsibilities

• Demonstrates competency with potential patients, hospice eligibility and meeting with families in assessment of their needs.
• Maintains confidentiality and safety of patient/client clinical records.
• Completes assigned admissions and patient care orders in an effective and timely manner.
• Communicates effectively with physician(s), referral sources, patient, family, and hospice interdisciplinary team regarding the new admission of a patient.
• Communicates effectively with operational, clinical and sales leadership regarding status of pending referrals.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
