Job summary

VIA Health Partners seeks an experienced Registered Nurse for hospice admissions and care planning. You will perform admission assessments, develop patient-centered plans, educate families, and coordinate with physicians and the interdisciplinary team at time of admission.

Strong documentation, infection control, and communication with care teams are essential. Ideal candidates have an RN license, CPR certification, and at least 2 years in hospice or palliative care, with computer proficiency

Qualifications

• RN license active and unrestrictive.
• CPR certification required; CHPN preferred.
• Computer proficiency for EMR and documentation.
• Experience in hospice, palliative care or related fields.

Responsibilities

• Conduct admission nursing assessments to identify patient/caregiver needs.
• Develop individualized plan of care with patient, family and interdisciplinary team.
• Educate patient and caregivers about plan of care and treatments.
• Coordinate with physicians, POD and other staff for timely admissions.
• Maintain accurate clinical documentation and medication profiles.
• Ensure infection control and safe, professional care delivery.
• Communicate effectively with external agencies and the care team.
