Job summary

VNA Health is seeking an Admissions Registered Nurse to initiate hospice patient admissions, conduct comprehensive assessments, and coordinate plans of care within patients’ homes and communities. The role emphasizes nursing judgment, family involvement, and collaboration with the RN Case Manager to ensure smooth transitions and comprehensive hospice services.

Qualifications

• Current and active California RN license.
• Bachelor’s degree preferred.
• Minimum one year acute care experience required.
• One year recent home health or hospice experience preferred.
• Bilingual – Spanish preferred, but not necessary.
• Current CPR certification.
• Licensed driver with insured automobile.
• Supervises home health aides/LVNs.
• Excellent observation, verbal and written communication skills.
• Ability to respond to life-threatening situations.

Responsibilities

• Provide accurate, efficient hospice admissions.
• Complete initial patient and family assessment for hospice care.
• Develop and document plan of care with patient/family involvement.
• Administer medications and treatments as prescribed.
• Counsel the patient and family on nursing needs at admission.
• Provide health instructions and safety guidance to patient and caregiver.
