Job summary

Compassus is seeking a Hospice Admission Registered Nurse to assess patient eligibility for hospice and coordinate care. You will work closely with families and the Inter-Disciplinary Team to develop and communicate initial plans of care, ensuring patient needs are addressed from day one.

The role requires an active RN license in the state, at least two years of RN experience (hospice preferred), and current CPR/BCLS certification.

• Two years of registered nurse experience

Responsibilities

• Assess patients for hospice eligibility and discuss services with patients and families.
• Collaborate with the Inter-Disciplinary Team to develop initial plan of care.
• Ensure initial patient needs are addressed and documented.
• Coordinate admissions and communicate with care teams for smooth transitions.
