Job summary

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED/ICU background preferred.
• Ability to communicate in English; Spanish bilingual preferred.
• Valid driver’s license and reliable transportation.
• Ability to work in a team and manage multiple priorities.
• Physical ability to perform job functions including lifting.

Responsibilities

• Completes clinical admissions for hospice services.
• Explains services to patients/families with empathy.
• Educates on admission requirements and initiates care plans.
• Obtains physician orders and documents per policy.
• Coordinates with physicians and care team on plans.
• Uses SBAR for timely clinical communication.
• Discharges planning and referrals to service lines.
• Assists during emergencies and after-hours as needed.
