Job summary

Compassus is seeking a Hospice Admission Registered Nurse (RN) to evaluate patient eligibility for hospice and coordinate initial care plans. The role emphasizes compassionate assessment, patient education, and collaboration with the care team to support families at home or in care settings.

Ideal candidates hold an active RN license, have hospice experience, and possess CPR/BCLS certification, plus a valid driver’s license to visit patients across communities.

Qualifications

• Must hold a valid RN license in the state of employment.
• Two years of RN experience; hospice experience preferred.
• Current CPR and BCLS certification.
• Valid driver’s license for community-based visits.

Responsibilities

• Assess patients for hospice eligibility and explain hospice care services to patients, families, and caregivers.
• Ensure all patient initial needs are addressed and documented.
• Collaborate with the Inter-Disciplinary Team on initial plan of care and patient condition.
