Job summary

VIA Health Partners, a leader in end-of-life care, is seeking a registered nurse to coordinate and deliver patient/caregiver care in our hospice program. You will conduct nursing assessments, develop and update care plans, and educate patients and families while ensuring continuity of care.

You will communicate with the IDG, be mindful of infection control, and maintain accurate clinical documentation. A compact state license, CPR, and 1–2 years of relevant experience are expected; CHPN is

Qualifications

• RN license in good standing and active in home state or compact state.
• Current CPR certification is required; CHPN preferred.
• Minimum 1 year inpatient (IPU) experience; 2 years for other nursing roles.

Responsibilities

• Deliver and coordinate patient/caregiver care.
• Conduct nursing assessments and update care plans.
• Educate patients and families; document promptly.
• Maintain infection control standards and complete paperwork.
• Participate in IDG conferences and coordinate referrals.
