Qualifications

• Diploma, ADN, or BSN in nursing from an accredited institution.
• Current Maryland nursing license.
• Minimum of three years of nursing experience; home health or hospice preferred.
• Ability to educate patients and families and work with a variety of teams.

Responsibilities

• Completes initial patient/family assessment to determine hospice eligibility and needs.
• Prepares and implements the initial care plan with Medical Director approval.
• Provides direct nursing care to maintain high-quality hospice care.
• Communicates findings to the team and coordinates with social services and other departments.
• Educates family members and caregivers on care tasks, medications, and safety.
• Ensures home equipment needs are arranged and supports discharge planning as needed.
• Participates in process improvement and maintains relationships with referral sources.
