Job summary

Greater Baltimore Medical Center (GBMC) seeks a Hospice Home Care Nurse who identifies patient and family care needs and provides nursing and supportive care for terminally ill patients and their families. Responsibilities include initial assessments, care planning with an interdisciplinary team, and direct nursing care in home settings.

The role requires a valid Maryland RN license, ADN/BSN (Diploma acceptable), and about two years of nursing experience or equivalent education.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• Two years of nursing experience, or equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs—physical, emotional, spiritual and mental status.
• Prepares and implements the care plan for patient/family unit in collaboration with the Interdisciplinary Group.
• Provides direct nursing care to patients in order to maintain highest level of care with the primary physician.
• Plans and supervises the delivery of care by the Home Health Aide (HHA).
• Updates primary physician and interdisciplinary team on a monthly basis and at care conferences.
• Teaches family members and caregiving staff patient care skills, including medications, injections, IV management, dressings, and vital signs.
