Qualifications

• Active RN license in Maryland.
• Three years of nursing experience; hospice/home health experience preferred.
• Education: Diploma, ADN and/or BSN from an accredited program.

Responsibilities

• Perform initial patient and family assessment to determine hospice eligibility and needs.
• Prepare and implement the initial care plan for patient/family with Medical Director approval.
• Conduct admission visits, level-of-care visits, pre-scheduled visits, and on-call visits as needed.
• Provide direct nursing care to maintain the highest level of care.
• Document clinical findings to support hospice eligibility and discharge planning.
• Educate family members or hired caregivers on patient care skills and safety.
• Arrange for home equipment and supplies as needed.
• Participate in department projects and process improvements.
