Job summary

Trinity Health is seeking a Hospice Admissions Registered Nurse (RN) to coordinate and complete hospice admissions across the Columbus market. The role requires clinical expertise, assessment of eligibility, and an individualized care plan to ensure a smooth transition into hospice services.

The RN collaborates with patients, families, physicians, and the interdisciplinary hospice team to promote comfort, dignity, and quality of life, while supporting referral sources and maintaining accurate

Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) license.
• Minimum one year of acute care nursing experience.
• Hospice, palliative care, home health, oncology, critical care, or medical-surgical experience preferred.

Responsibilities

• Conduct comprehensive hospice admission assessments to determine patient needs and eligibility.
• Educate patients and families about hospice philosophy, benefits, services, and care expectations.
• Obtain informed consent and complete admission documentation.
• Develop and initiate the individualized Plan of Care (POC) with the patient, family, physician, and team.
• Coordinate with referral sources, physicians, hospitals, and facilities to facilitate admissions.
• Maintain accurate documentation and synchronize records after each patient visit.
• Provide nursing assessments and caregiver education.
• Participate in admissions rotation and on-call activities.
