Job summary

Hospice of South Texas is seeking a compassionate RN Admission Nurse to complete hospice admissions and provide skilled nursing care in patients' homes. This role serves as a clinical liaison between patients, families, referral partners, and the interdisciplinary team to ensure timely admission, care coordination, and regulatory compliance.

The role includes coordinating with the IDT, educating families, and ensuring documentation and compliance.

Qualifications

• Graduate of an accredited nursing program and Texas RN license.
• Driver's license and reliable transportation.
• Hospice/home health experience preferred.

Responsibilities

• Perform comprehensive admission assessments addressing physical, functional, psychosocial, spiritual, and environmental needs.
• Coordinate care with the interdisciplinary team and referral partners.
• Educate patients and families on disease progression, symptom management, and end-of-life expectations.
• Complete and submit all documentation timely per HOST policies and regulatory standards.
