Job Juncture in Kansas City, MO is seeking an Admissions RN for a full-time role with a 50-mile radius from the office. The nurse will coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and work with the interdisciplinary team to ensure smooth transitions into hospice services.

Required qualifications include MO RN license, driver's license, and hospice experience.

Qualifications

• Active MO Registered Nurse license.
• Current driver's license.
• Two years RN experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and auto liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsabilités

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time; perform other duties as assigned.

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities
