Job summary

H.T. Prof Executive Search seeks an experienced Admissions RN for a full-time role in the Kansas City metro area (MO).

You will coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and partner with the care team to ensure smooth transitions into hospice services. Responsibilities include head-to-toe assessments, timely documentation, and collaboration with patient care teams to maintain continuity of care.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver's license.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.
• Reliable transportation and valid automobile liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational, and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.
