Unknown Hospice Provider in Kansas City Metro, MO (east) is seeking a dedicated Admissions Registered Nurse for a full-time role with a 50-mile radius from the office. The position entails coordinating new patient admissions, conducting comprehensive assessments, and educating patients and families while working with the interdisciplinary care team to ensure a smooth transition to hospice services.

Requirements include Missouri RN licensure, CPR, a valid driver's license, and 2+ years of RN

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current CPR certification.
• Two years of RN experience in a clinical setting.
• One year of full-time hospice experience.
• Current driver's license and reliable transportation.
• Ability to work independently and make sound clinical decisions.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills
