Job summary

6 Degrees Search is seeking a dedicated Hospice Admissions RN for the Kansas City Metro, MO area. This full-time role coordinates new patient admissions, conducts comprehensive assessments, educates patients and families, and partners with the interdisciplinary care team to ensure a smooth transition into hospice services.

The ideal candidate holds a Missouri RN license, CPR certification, and hospice experience.

Qualifications

• Active Missouri RN license is required.
• Two years of RN experience in a clinical setting.
• One year of full-time hospice experience.
• Current driver's license and reliable transportation.
• Ability to work independently and make clinical decisions.
• Strong written and verbal communication skills.

Responsibilities

• Coordinate all aspects of new patient admissions.
• Collaborate with care teams to ensure smooth communication.
• Educate patients and families on condition and care needs.
• Perform head-to-toe physical assessments.
• Document accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities

• Coordinate and oversee all aspects of new patient admissions
• Collaborate with patient care teams to support seamless communication and continuity of care
• Educate patients and families regarding condition, needs, and care
• Perform head-to-toe physical assessments
• Complete all required documentation accurately and on time
• Perform other duties as assigned
