Job summary

The Keystone Distinction Group is seeking a dedicated Hospice Admissions RN in the Kansas City metro (east) MO area. This full-time role requires coordinating new patient admissions, conducting comprehensive assessments, educating patients and families, and partnering with the interdisciplinary team for smooth transitions into hospice services.

Ideal candidates hold an MO RN license, CPR certification, and have hospice experience or strong willingness to learn.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Driver's license valid for travel within territory.
• CPR certification current and BLS-compliant.
• Two years RN experience in a clinical setting.
• One year hospice experience preferred but not required.
• Reliable transportation and vehicle insurance.
• Ability to work independently with sound clinical judgment.
• Strong written, verbal, organizational and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to ensure continuity of care.
• Educate patients and families regarding condition and needs.
• Perform head-to-toe physical assessments.
• Complete required documentation accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities

• Coordinate and oversee all aspects of new patient admissions
