Job summary

Aspire Talent Solutions is seeking a dedicated Hospice Admissions RN for the Kansas City metro area (east), full-time. This role coordinates new patient admissions, conducts comprehensive assessments, educates patients and families, and partners with the care team to ensure a smooth hospice transition.

The successful candidate will have an active Missouri RN license, current CPR, a valid driver’s license, and hospice experience.

Qualifications

• Active Missouri RN license.
• CPR certification current.
• Valid driver’s license and reliable transportation.
• Minimum 2 years RN experience in clinical setting.
• Minimum 1 year hospice experience (FT).
• Ability to work independently and make clinical decisions.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to ensure continuity of care.
• Educate patients and families about condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Kansas City Metro, MO (east) | Full-Time

Territory / Work Schedule: 50-mile radius from the office, schedule Tuesday-Saturday 10am-7pm

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities

• Coordinate and oversee all aspects of new patient admissions

• Educate patients and families regarding condition, needs, and care
