Job summary

Zaddy Solutions, serving the Kansas City metro, seeks a full-time Hospice Admissions RN to coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and partner with the interdisciplinary care team to ensure a smooth hospice transition.

The role covers a 50-mile radius from the office with a Tuesday-Saturday 10am–7pm schedule. MO RN license, CPR, and hospice experience are required; strong clinical judgment and communication skills are essential.

Qualifications

• Missouri RN license active and in good standing.
• Current driver's license and CPR certification are required.
• Minimum two years RN experience in a clinical setting.
• At least one year of full-time hospice experience.
• Reliable transportation and automobile liability insurance.
• Ability to work independently with sound clinical judgment.
• Strong written, verbal, organizational and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.
• Perform other duties as assigned.

Hospice Admissions RN

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities
