Descrizione del lavoro

GBMC HealthCare is seeking a Hospice Admissions Nurse to assess patients in their homes or hospitals and prepare them for hospice services. The role serves as an ambassador for GBMC within the service area, with weekend shifts and a bonus structure.

The position requires an ADN/BSN, current MD nursing license, and 3 years of nursing experience. Knowledge of infection control, EMR, and strong communication are essential. Eligible for full-time benefits and GBMC values guide daily work.

Competenze

Three years of nursing experience; Home Health or Hospice preferred but not required.
Diploma, ADN and/or BSN from an accredited institution.
Current MD nursing license.
Infection control practices and patient safety focus.
Proficient in EMR and patient data documentation.

Mansioni

Completes initial assessment of patient and family to determine hospice eligibility and needs.
Prepares initial care plan for patient/family unit with Medical Director approval.
Performs admission visits, level-of-care visits, and on-call visits as needed.
Provides direct nursing care to maintain highest level of care.
Document clinical findings to support hospice eligibility and discharge planning.
Educates patient, family, or caregivers on care tasks and symptom management.
