Job summary

Affinity Health Management is seeking an Admissions Nurse to join our hospice care team. You will perform skilled nursing assessments, plan care on an on-call basis, and coordinate with patients, families, and physicians to begin appropriate care at admission.

As part of the interdisciplinary team, you will implement the plan of care, provide caregiver education, administer prescribed treatments, and ensure a smooth transition into hospice services while maintaining compassionate,

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in Ohio in good standing.
• One year of professional nursing experience in the last three years; BSN with two years' public health/home care/hospice experience preferred.
• Background checks and references as required.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide nursing care using the nursing process.
• Write and initiate the plan of care and revise it as patient needs change.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in meeting nursing and related needs.
• Provide health instructions and assist with activities of daily living.
• Coordinate patient care as Case Manager when assigned.
• Participate in on-call duties and ensure equipment and services are available.
• Collaborate with the interdisciplinary team and hospice staff to ensure quality care.
