Qualifications

• Initial, comprehensive patient and family assessment for hospice need.
• Nursing process-based professional care.
• Develop and revise care plans with patient and family.
• Coordinate care with Clinical Supervisor and interdisciplinary team.
• Provide patient and family education and guidance.
• Schedule includes on-call nights and PRN visits.

Responsibilities

• Complete initial and ongoing assessments of patient and family.
• Provide nursing care using the nursing process.
• Develop and revise plans of care based on patient needs.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing needs and self-care.
• Coordinate with physician and interdisciplinary team.
