Advocate Health is seeking a Hospice Registered Nurse to lead the interdisciplinary care team, coordinate patient plans, and educate patients and families about hospice services within the Advocate Health system. Requirements include an active RN license, Associate's Degree in Nursing, and BLS certification.

Qualifications

• RN license in the state where the team practices.
• BLS for Healthcare Providers certification (AHA) may be obtained within 6 months.
• Associate's Degree in Nursing is required.

Responsibilities

• Admit patients to hospice services and lead the interdisciplinary care team (IDT) for each patient and family.
• Obtain patient history, complete assessments, and evaluate appropriateness for hospice services and level of care.
• Explain hospice services and obtain informed consent; verify verbal prognosis with the IDT and medical director.
• Review insurance/Medicare eligibility and document data to support billing efforts.
• Coordinate services with IDT; arrange equipment, supplies, and services; ensure proper handoff communication.
• Facilitate appropriate placement in hospice contracted facilities when needed.
• Assess delivery of medications and home medical equipment; ensure timely availability.
• Provide hospice education to inpatient hospital/facility staff and serve as expert resource.
