Requires an active RN license in the state, 1+ year nursing experience (hospice/hospital preferred), and EMR proficiency. English communication is essential; Spanish bilingual preferred.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED/ICU background preferred.
• English communication; Spanish bilingual preferred.
• Valid driver's license and insured vehicle.

Responsibilities

• Completes clinical admissions for all company services and explains services empathetically.
• Identifies objections and answers questions; educates patients and families on admission requirements.
• Obtains physician orders and initiates care plans addressing immediate needs.
• Communicates with Physicians, Care Navigation, and facilities; coordinates referrals.
• Performs skilled nursing interventions to prevent complications and maximize comfort.
• Ensures safe discharge planning and accurate documentation per policy.
• Maintains SBAR communications and supports after-hours staff as needed.
• Represents the company professionally and complies with regulations and policies.
