Qualifications

• Bachelor’s degree in nursing or equivalent experience.
• Current RN license in the state of practice.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Strong clinical assessment skills; ICU/ED background preferred.
• Experience with EMR/EHR systems.
• English required; bilingual in Spanish preferred.
• Valid driver’s license and reliable transportation.
• Ability to communicate professionally and work in a team.
• Capacity to manage multiple priorities and handle emotional stress.
• Active BLS for healthcare professionals.

Responsibilities

• Completes clinical admissions for all Company services including, but not limited to: explains services to patients/families and addresses questions; presents services empathetically.
• Identifies admission objections or barriers and provides responses.
• Educates patient/family on admission requirements and immediate needs.
• Obtains physician orders and initiates Plan of Care for needs.
• Initiates nursing interventions to prevent complications and comfort patients.
• Coordinates with Care Navigation for timely follow-up.
• Maintains SBAR communication and prepares for next assignment.
• Develops relationships with case managers and facilities.
• Works with PRRs to educate on available services.
• Performs other duties as assigned.
