Job summary

Chapters Health System is seeking an Admission RN to assess patients in residences or hospitals and initiate care within our system. You will ensure a seamless transition to Care Navigation and serve as an ambassador for the organization.

The role requires current RN license, 1+ year nursing experience (hospice/hospital preferred), EMR/EHR proficiency, and strong clinical assessment. A valid driver's license and bilingual Spanish are advantageous.

Qualifications

• Current RN license in the state of employment.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED or ICU background preferred.
• Ability to communicate in English (written and verbal); bilingual Spanish preferred.
• Mobile Driver with valid license and automobile insurance per policy.
• Reliable transportation to meet schedule.
• Ability to work in a team and manage multiple priorities.
• Ability to perform essential functions including lifting and physical requirements with/without accommodation.

Responsibilities

• Completes admissions for all Chapters Health System services.
• Explains services to patients/families and addresses questions.
• Educates patients/families on admission requirements and initial assessment.
• Initiates skilled nursing interventions to prevent complications and maximize comfort.
• Obtains physician orders and initiates Plan of Care.
• Liaises with care navigation and facilities to ensure timely follow-up and referrals.
• Communicates via SBAR and coordinates with after-hours staff.
• Arranges safe discharge and gathers medical record information.
• Develops relationships with case managers and physicians.
