Chapters Health is seeking an Admission RN to assess patients in their homes or in hospitals and initiate care within our services. You will coordinate with Care Navigation and act as an ambassador for the organization.

The ideal candidate holds an active Florida RN license with at least 1 year of nursing experience; hospice or hospital background is preferred. A strong English communicator, Spanish bilingualism is a plus. Sign-on bonus and travel reimbursements are available.

Qualificações

• Active RN license in Florida.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills.
• English fluency; Spanish preferred.

Responsabilidades

• Complete admissions for all company services.
• Explain services to patients and families with empathy.
• Educate on admission requirements and initiate care plans.
• Coordinate with Care Navigation for timely follow-ups.
• Represent the company professionally at facilities.
