Job summary

Agape Care Group is seeking a Hospice Account Executive to serve as the main liaison between our hospice agency and local healthcare providers, driving referrals and access to compassionate end-of-life care.

You will build relationships with hospitals, physicians and referral partners, educate clinicians about services, and coordinate with clinical teams to ensure smooth patient transitions across our care network.

Qualifications

• Heart to serve patients and families.
• Associate degree or proven work experience in health-related field; RN/LPN preferred.

Responsibilities

• Build and maintain relationships with hospitals, physicians, case managers and referral partners.
• Generate hospice referrals within assigned territories and existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams for seamless patient transitions into hospice care.
• Identify new market opportunities and strengthen community partnerships.
• Maintain accurate referral documentation and communication with care teams.
