Qualifications

• Reliable transportation and ability to travel daily

Responsibilities

• Serve as liaison between hospice agency and local healthcare providers
• Generate hospice referrals in assigned territories and existing accounts
• Educate healthcare professionals about hospice services and patient eligibility
• Coordinate with clinical teams to ensure seamless patient transitions into hospice care
• Identify new market opportunities and strengthen community partnerships
• Maintain accurate referral documentation and communication with care teams
