Job summary

The Salvation Army North & Central Illinois Division Harbor Light Center (HLC) is seeking a Case Manager to provide individualized case management, supporting adults toward housing and employment goals. You will maintain records, recruit participants, and promote social services in the Minneapolis area.

Responsibilities include developing service plans, outreach activities, and collaboration with community partners to connect participants with resources.

Qualifications

• Bachelor’s degree in a relevant field; a combination of education and experience may be considered in place of a degree.
• State board licensure is required if the degree is in social work.
• Previous experience in social services is required.
• Experience with long-term case management is preferred.
• Must successfully pass all required background checks.
• A valid driver’s license is preferred, and candidates must meet the MVR requirements.

Responsibilities

• Provide short- and long-term, strength-based case management for adult participants achieving housing, employment, and self-sufficiency goals.
• Develop and implement individualized service plans and monitor progress.
• Recruit eligible participants for housing programs through outreach and referrals.
• Promote social services and Corps programs to increase engagement and accessibility.
• Maintain accurate participant records in case files and databases with timely, compliant notes.
