Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations across a defined coverage area. You'll travel to patients' homes up to 100% time, conduct medical history reviews, physical evaluations, and diagnostic tests as needed, using an iPad and clinical workflow.

This role emphasizes building trusted relationships, flexibility to travel, and multi-state licensure as required.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses as required.

Responsibilities

• Provide in-home health evaluations with patient history review and physical examination.
• Conduct diagnostic tests as ordered by health plan (e.g., spirometry, others).
• Travel across defined coverage area; up to 100% travel may be required.
• Maintain clinical workflow using iPad and approved systems.
• Build trusted relationships to improve health outcomes for patients.
