Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations across a defined coverage area. The role involves extensive travel, home-based assessments, and coordination with health plans. Licensure across multiple states is required or attainable as dictated by business needs.

The clinician will perform comprehensive evaluations using an iPad workflow, including history, physical exams, and selective diagnostics, with virtual visits when needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

• Be flexible to travel locally and within licensed states.
