Job summary

Signify Health, part of CVS Health, seeks a Full Time Physician to provide in-home health evaluations, engaging with people in the comfort of their homes.

The role requires up to 100% travel across licensed states, multiple-state licensure, and a 40-hour work week, using an iPad with a clinical workflow.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Conduct in-home health evaluations including med history and physicals.
• Coordinate with health plans and provide diagnostic services as ordered.
• Travel locally and nationally to reach members.
• Utilize iPad-based clinical workflow during visits.
• Support patients and families with care connections.
