Job summary

Signify Health, part of CVS Health, seeks Full Time Physicians to provide in-home health evaluations. You will travel across a defined coverage area, performing comprehensive health assessments using an iPad workflow.

The role emphasizes building trusted relationships and bridging care gaps for plan members. Requires active licenses and multi-state capabilities; travel is expected, with potential overnight stays.

Qualifications

• Active, unrestricted MD/DO licenses in coverage area(s).
• Able to obtain and maintain multi-state licenses as required by coverage areas.
• Experience or ability to conduct in-home health evaluations and coordinate with health plans.

Responsibilities

• Provide in-home health evaluations for members at their residences.
• Travel across a defined coverage area, with up to 100% travel and potential overnight stays.
• Use an iPad and clinical workflow to perform evaluations.
• Perform diagnostic and preventive services as indicated by health plans (e.g., tests).
• Conduct virtual visits as needed and coordinate with care teams.
