Job summary

Signify Health, part of CVS Health, seeks a Full-Time Physician to conduct in-home health evaluations across a defined coverage area. The role involves 45‑minute visits, medical history review, physical exams, and optional diagnostic tests per health plan orders.

You will travel locally and within licensed states, using an iPad workflow and a collaborative care team to connect members to the right care and services.

Qualifications

• MD or DO with active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses as required.
• Willingness to travel up to 100% to defined coverage areas.

Responsibilities

• Conduct in-home health evaluations with patients, including medical history review and physical assessment.
• Provide diagnostic and preventive services as indicated by health plans; perform virtual visits when needed.
• Engage with health plan members to connect them to appropriate care across licensed states.
• Work within Signify Health clinical workflow using an iPad-based system.
