Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide in-home health evaluations across a defined coverage area. Travel up to 100% may be required, with potential overnight stays and licenses in multiple states.

You will perform 45-minute visits, review medications and medical history, conduct physical evaluations, and coordinate with health plans to connect patients with the right care. Virtual visits may be part of the role.

Qualifications

• Active, unrestricted medical license in coverage areas.
• Able to obtain and maintain multi-state licenses as required.

Responsibilities

• Provide in-home health evaluations with comprehensive clinical assessments.
• Review medications and medical history, perform physical evaluations.
• Coordinate with health plans to facilitate appropriate care access.
• Travel across defined coverage area; may require overnight stays and flights.
• Conduct virtual visits as needed and support diagnostic services when ordered.
• Engage with patients and families to explain findings and next steps.
