Job summary

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations. You will travel up to 100% across defined coverage areas, use an iPad with a clinical workflow, and may require overnight stays or flights based on geography.

Hold multiple state licenses as required. You’ll build trusted relationships to connect people with the right care, conducting 45-minute visits including medication and history reviews and physical evaluations.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Able to obtain and maintain multi-state licenses in coverage area, as required.

Responsibilities

• Visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs.
• Conduct in-home visits with comprehensive health evaluations using an iPad and clinical workflow.
• Provide all Signify Health services as indicated including diagnostic and preventive services as needed.
• Be flexible to travel locally and within licensed states.
