Optimal Care is seeking a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric patients, often in homes or care settings, within a high-travel service area in Michigan. You will have clinical autonomy and work within a collaborative practice to improve comfort, function, and quality of life.

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation and valid driver’s license.

Responsabilités

• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications as part of patient care.
• Coordinate closely with physicians, caregivers and the inter-disciplinary team.
• Serve as a clinical decision-maker and help patients/families understand diagnoses.
• Aim to manage chronic illness and reduce hospitalizations through coordinated care.
