Job summary

Optimal Care seeks a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with the autonomy to focus on high-quality, patient-centered care that improves comfort and function.

You will perform comprehensive evaluations, manage treatment plans, prescribe medications, and work closely with physicians and caregivers in a collaborative practice to reduce hospitalizations and support

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.

Responsibilities

• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications and coordinate with physicians, caregivers, and the interdisciplinary team.
• Help patients and families understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.
