Optimal Care is seeking a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric patients, often in homes or care settings. You will evaluate patients, develop individualized plans, prescribe medications, and coordinate with physicians and caregivers in a collaborative practice.

This high-travel role serves Saginaw, MI and surrounding areas, with weekday hours and a focus on meaningful patient relationships and quality of life.

Qualifications

• Master's or doctoral degree and board certification as NP/PA.
• Active state license as NP or PA.
• Reliable transportation and valid driver’s license.

Responsibilities

• Conduct comprehensive medical evaluations and develop treatment plans.
• Prescribe and monitor medications; coordinate with physicians and caregivers.
• Manage chronic illness and help reduce unnecessary hospitalizations.
• Collaborate within a clinical team to ensure patient-centered care.
