Job summary

LiveWell Homecare Agency, LLC is seeking an experienced NP/PA to conduct in-home health assessments for adults and geriatrics, reviewing medical history, medications, social factors, and health risks during visits that can last up to an hour.

The role involves coordinating with healthcare teams, administering tests, and delivering education and follow-up at members’ residences, including homes and facilities. Travel is extensive, with licensure and certifications required.

Qualifications

• Master’s degree or equivalent experience in a relevant field.
• Current NP/PA licensure with prescriptive authority and board certification.
• At least one year of experience as NP/PA.
• Strong communication skills, computer proficiency, and extensive travel required.
• Valid driver's license and appropriate insurance coverage required.

Responsibilities

• Perform adult/geriatric evaluations and comprehensive assessments lasting up to one hour.
• Review medical history, medication adherence, social factors, and health risks.
• Administer tests and collaborate with healthcare teams.
• Provide education and follow-up at members’ residences, including homes and facilities.
