Job summary

NPHire is hiring a Nurse Practitioner for local, in-home patient visits in King County, WA. You will conduct assessments, manage medications, and coordinate care with primary providers within 8-hour daytime shifts across counties.

The role requires 1+ year of clinical experience, a valid Washington NP license by start date, and active ANCC/AANP certification. Strong communication and EMR skills are essential for delivering high-quality home health care.

Qualifications

• At least 1 year of clinical experience in family, geriatric, primary care, home health, or house-call settings.
• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license in Washington or ability to obtain by start date.
• Active driver's license and reliable transportation for in-home visits.

Responsibilities

• Conduct comprehensive annual health assessments in patients' homes.
• Review histories, reconcile medications, obtain vitals, and perform focused exams.
• Complete screenings and point-of-care testing as appropriate.
• Document diagnoses to support care management and treatment planning.
• Coordinate with primary care providers and educate patients on disease management and prevention.
• Address social determinants of health and connect patients with community resources.
• Identify urgent needs and escalate care when necessary.
• Document visits accurately in the electronic medical record.
• Collaborate with care teams to keep patients healthy at home.
