UnitedHealthcare is seeking a Nurse Practitioner for Home and Clinic in Shreveport, LA. You will conduct clinical assessments, perform exams, document in EMR, and coordinate care to improve member outcomes with travel to member homes as needed.

The role requires an active RN license in Louisiana, NP certification (ANCC/AANP/AACN), prescriptive authority, and ability to travel to diverse environments. In-home visits and geriatric care experience are a plus; strong documentation and communication

Qualificações

• Active nursing license to practice in Louisiana
• 1+ years of clinical experience in highest level of education
• Nurse Practitioner Certification from ANCC, AANP or AACN
• Prescriptive authority in Louisiana
• Willingness to travel to member homes and work in diverse environments
• Ability to lift 30-pound bags and navigate stairs

Responsabilidades

• Conduct assessments in member homes and clinics with accurate documentation
• Check vitals and perform physical exams including foot/monofilament tests
• Identify diagnoses for care management
• Document health conditions and communicate gaps in care to PCP
• Educate members on diseases, medications and adherence
• Initiate referrals as needed
• Comply with HIPAA and protect PHI
• Travel to member sites and work autonomously
